Job Description

As part of our ongoing commitment to improve the discharge process for patients transferring to Residential and Nursing Homes following a stay in acute hospitals, we are looking to appoint a Bank Care Home Trusted Assessor based at Kettering General and Northampton General Hospital

You will support the current team to enable residents residing or needing to enter a Care Home to fully assessed as part of the discharge planning alongside the integrated team of Health and Social Care Staff. the role required the Assessor to liaise with local residential and nursing homes, including visiting them as needed.

We are looking to appoint a motivated recruit to support the current team and provide cover for leave, sickness and increase demand. You may have a nursing qualification but will need to have the knowledge and experience on a senior or management level within elderly residential social care. The objective of the role is to ensure and support the safer hospital discharge as a member of the National Care Home Trusted Assessor team.

Registered Nurse
Care Home Manager including Nursing
Worked in Elderly Social Care
Deputy Care Home Manager
At least 5 years' experience in health or social care

The salary is up to 17.00 per hour depending on experience.
