Job Purpose
The Trusted Carers Assessor will proactively identify, engage, and support unpaid carers in Luton, ensuring they can access Carers Assessments, information, and early support. The role focuses on outreach, early identification, and engagement with seldom heard communities, in line with the Luton Carers Strategy 2023 to 2028 and agreed service KPIs.
This role plays a key part in promoting carers' wellbeing, independence, and access to timely support while ensuring accurate data recording, safeguarding, and partnership working with the funder.
Key Responsibilities:
Outreach and Engagement
Proactively engage unpaid carers across Luton through community outreach, events, GP surgeries, community venues, and digital engagement.
Develop culturally sensitive approaches to reach under-represented groups, including LGBTQ+ carers, faith communities, and ethnic minority groups.
Support and encourage carer involvement in co-production activities, including working with DRC Experts by Experience.
Carer Identification and Support
Support carers to identify themselves and provide consented contact details for recording on the DRC CRM system.
Increase the number of newly identified carers each month in line with service KPIs.
Provide accessible information on carers' rights, local services, financial entitlements, and available support.
Signpost carers to appropriate services, including statutory services provided by Luton Council.
Promote carers' wellbeing, independence, and early support to prevent crisis situations.
Systems, Safeguarding and Quality
Accurately record carer information on the CRM system in line with GDPR and data protection requirements.
Escalate contacts for Carers Assessments where appropriate and support the carer journey in collaboration with the funder.
Ensure safeguarding concerns are identified and referrals are made promptly and appropriately.
Deliver all support in a culturally sensitive, inclusive, and person-centred manner.
Collaboration and Reporting
Report to the Operations Manager and contribute to effective partnership working with the funder.
Maintain a high standard of accuracy, integrity, and accountability in all work.
Actively contribute to service improvement, learning, and best practice.
Person Specification:
Essential
Experience in community outreach, engagement, or support roles.
Knowledge of carers' issues and statutory rights, including the Care Act 2014.
Excellent verbal and written communication skills.
Ability to build rapport with people from diverse backgrounds.
Strong listening skills, empathy, and understanding.
Understanding of confidentiality, GDPR, and data protection.
Basic IT skills, including CRM systems and data entry.
Commitment to inclusion, dignity, and respect.
Desirable
Experience working with carers, within health and social care, or lived experience as an unpaid carer.
Previous experience or training as a Trusted Assessor.
Ability to facilitate group sessions and co-production activities.
Knowledge of local services in Luton and culturally sensitive outreach practices.
Ability to speak additional community languages relevant to Luton.
Experience using social media to engage seldom heard groups.
Knowledge of relevant legislation, including the Equality Act 2010, Care Act 2014, and Health and Social Care (Safety and Quality) Act 2015.
