Job Description

The Virtually Assessed Pathway clinic (VAP) is an award winning service, unique to Velindre Cancer Centre, This post holder will be joining a small team of nurses and pharmacy technicians led by independent prescribers. The main aspect of the role will be to work, as part of a team, within the telephone assessment clinic to clinically assess a patients fitness to proceed with the next cycle of SACT (Systemic Anti Cancer Treatment) using a clinically robust assessment form, under the supervision of an independent prescriber.



The VAP service is not just a pre SACT assessment clinic, we manage the patients' cancer pathway and as a result we become part of their journey.



The VAP clinic currently works with the following teams assessing patients on suitable regimens from the following cancer sites



Colorectal / Breast / Urology / Gynae / Skin/ UGI / Hepatobiliary/ Lung.



The post holder is responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care, participating and contributing to the wider clinical governance agenda including service developments and flexible working to meet the needs of patients.



The working week hours can be flexible. This job can be part time / full time/ job share. We can consider compressed weekly hours.



The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.



To work with patients and their carers to determine in partnership

the assessment of patient needs. Working collaboratively with the

multi disciplinary/multi agency team.

To liaise and provide guidance on your area of practice to inpatient

and outpatient on the care of patients when requested.

To act as the named nurse/ keyworker for designated individuals

To ensure that patients receive care according to plan,

delegating the care to be given to other members of the team as

appropriate.
