Weekend Supported Housing Worker
Salary:
8,075 per year (full-time equivalent 25,235) | hourly rate of 12.94
Hours:
12 hours per week

Somerset East & Somerset (mainly Frome & Shepton Mallet)
Additional information:
Valid driver's license and access to a car business purposes
Participation in an on-call, out-of-hours rota (100 per week when on-call)
Standard working days are Saturday and Sunday
The Role:
As a Support Worker, you'll play a key role in providing person-centred support and housing management to individuals who have experienced rough sleeping and have high support needs. Working across four dispersed units, you'll build strong relationships with residents to encourage engagement, reduce barriers, and support them in maintaining their tenancy while working towards greater independence.
You'll work closely with the weekday Support Worker to ensure a seamless and consistent approach, helping an average of four clients on their journey towards stability and a brighter future.
Responsibilities include:
Progress and track referrals into the service and outcomes for clients moving on from the service
Complete holistic assessments for clients, leading to the development of support plans which meet individual client goals
Give clients advice, and information and provide an enabling service so that they can be signposted to and access appropriate services to develop independence
Embed client involvement in all aspects of the support we provide and create a culture within service delivery which ensures the clients voice and feedback is encouraged, and heard
Maintain client records that they are accurate and fully updated using our client record system
Since job descriptions cannot be exhaustive; the post holder may be required to undertake other duties, which are broadly in line with the above key responsibilities
Qualifications / Requirements:
A valid UK driving licence and access to own vehicle - business insurance will be required for roles involving travel for work
Experience working with vulnerable adults and/or people with complex needs and challenging behaviours
