Job Description

Care Team Leader is responsible for supervising care staff, ensuring high-quality care delivery, and supporting residents' needs in a healthcare or social care setting.

Key Responsibilities

Supervision and Leadership

: Lead and supervise a team of care staff, ensuring they provide high-quality care and support to residents. Act as a role model for best practices in care delivery.

Care Planning

: Develop, implement, and review personalized care plans in collaboration with residents, their families, and healthcare professionals. Ensure that care is tailored to meet individual needs.

Training and Development

: Provide training and support to new staff members, ensuring they understand their roles and responsibilities. Conduct regular supervisions and appraisals to promote professional development.

Quality Assurance

: Monitor and evaluate the quality of care provided, ensuring compliance with regulatory standards and best practices. Address any issues or concerns promptly.

Communication

: Maintain effective communication with residents, families, and healthcare professionals. Ensure that all relevant information is documented accurately and shared appropriately.

Health and Safety

: Ensure that all health and safety policies are adhered to, promoting a safe environment for residents and staff. Conduct risk assessments and implement necessary measures to mitigate risks.
