Working closely with GP practices and the wider ICC team, the Wellbeing Activator will work within the Wellbeing Service supported by the Senior Living Well coach, and Living Well Coaches, provide training, devise, develop and implement Wellbeing Groups for individuals who present with Long Term Conditions (LTCs), Frailty and psycho-social needs engaging them to set personalised goals to support self-management. This post will provide line management to the Senior Living Well Coach and group supervision for the Living Well Coaches.
The postholder will have skills in motivational interviewing and appropriate psychological care skills. Training will be provided to enhance or develop additional skills in Cognitive Behaviour Therapy (CBT), Acceptance & Commitment Therapy (ACT) and mindfulness. Training will be provided within the Physical Health Psychology Department of North Cumbria Integrated Care Trust (NCIC). Ongoing clinical supervision and line management will be provided by the Consultant Counselling Psychologist. The role will include the establishment, planning, delivering, monitoring and evaluating "wellbeing" groups. These groups will be delivered alongside the LWC where appropriate. A rolling programme of groups run within GP practices or local community venues will cover topics such as emotional wellbeing, worry, and mood; mindfulness; fatigue and sleep; pacing and exercise; bereavement and loss; understanding pain; lifestyle and health.
