Salary: 12.83 per hour



Annual Leave: 20 days + Bank Holidays (pro-rata)



Working Hours: Mon-Fri 9am-4.30pm (part-time available too).



No Personal care involved.

This role is to work within a pool of Community Support Workers providing individual and group support to clients with an acquired brain injury. The post does not involve personal care The post is subject to enhanced Disclosure and Barring Service clearance.
Provide high quality, person-centred rehabilitation and reablement support to clients with an acquired brain injury at community venues, in client's own homes and in other community settings. Promote choice and control, personal wellbeing, independence, improved quality of life, social interaction, inclusion and peer support.

Liaise effectively with clients, family carers, Headway teams and other practitioners in helping clients progress to meet support plan goals for their independence. Take on the role of individual client link worker to enhance quality of engagement and personal support.

Provide client support one to one and in group and workshop activities both in person and online. Work effectively with Headway volunteers within a combined staff and volunteer team and jointly with other practitioners and partner colleagues furthering the charity's aims and values.

Facilitate and lead wellbeing support and reablement aspects of Living Well Programme services. Contribute routinely to Headway's digital and social media programme, encouraging client and carer involvement and feedback.

Client Wellbeing Support

Support clients face to face at Headway venues and via outreach in their own home and community and online and via the phone as preferred. Build effective working relationships and enhance individual support as a designated link worker. Support and lead/facilitate group activities, including online activity and discussion groups, encouraging full participation and engagement. Contribute to client assessment, preparation of support plans and regular reviews. Promote choice and control, independence, positivity and focused rehabilitation and reablement for clients. Help each person progress in achieving their individual goals and aspirations Involve clients in all decisions about their day-to-day support. Promote personal wellbeing, consistently providing a person-centred approach in all communications and activities. Liaise effectively with preferred partner providers and practitioners from a range of health, social care and community organisations, including local health and wellbeing teams. Provide one to one reablement support with client activities indoors, outdoors and online as part of individual support plans. Enable personal independence in daily living activities such as assisting with mobility or preparation of refreshments at community venues as required. Support clients to undertake activities in and around their home such as managing correspondence, cooking, cleaning, using public transport, shopping or in preparing daily and weekly routines and attending appointments. Accompany clients on Headway trips out where the purpose of the trip is to socialise with other clients or to provide respite for carers. Provide clients with continuous reassurance and routinely seek client and carer views and feedback, recording and reporting on this in line with Headway policy. Help clients to access other activities and support in the community as part of their individual support plans. Input client information activity data electronically onto the client database and on hard-copy records. Ensure accurate, high-quality data entry and comply with all data protection and consent requirements. Update the database to record client progress, changes or concerns. Help clients to access other activities and support in the community as part of their individual support plans. Participate in the Headway Helpline on rotation, responding to general enquiries and proactively calling back clients and other callers and take messages for Helpline lead. Work in partnership with all professionals, family carers, friends, volunteers, paid carers who may accompany clients and others involved with a client's rehabilitation and reablement to continuously help progress to improve quality of life. Wellbeing Support Workers may be asked to take on a particular Partnership Lead responsibility - for example Access to Physical Activity, Carer Support, Social Outings or Courses. Provide help and support at Headway events, promote excellent client/customer service. Help raise awareness of brain injury and the role of Headway as a specialist fundraising charity. Maintain and increase own knowledge of brain injury and best practice in the field of ABI support. Take-up continuing professional development opportunities including self-learning requested by the designated line manager to achieve a high standard of self-practice. Complete mandatory training and attend all planned Headway training days and staff meetings. Undertake the Care Certificate training (e-learning) as a standard Headway qualification. Complete this within an agreed specified time period subject to appropriate support, guidance and assessment by the line manager.
Other Duties

Respect and promote the dignity, rights, wishes and feelings of clients at all times and observe the charity's policies, aims and values-based culture. Identify risks in the workplace, in clients' homes and outside in community settings. Take immediate action to keep all individuals safe, and to remove and reduce risk. Report to the designated line manager on any further action required. Promote a working partnership and membership ethos. Help to achieve Headway business plan objectives and share thinking to help shape Headway's future. Support clients and carers in contributing their own accounts of lived experience including providing comments and feedback on Headway support. Actively encourage people to have their voices heard. Promote and implement Headway safeguarding procedures and report immediately to the line manager on any safeguarding concerns or issues. Promote health and wellbeing of every individual client. Create opportunity for clients to participate in activities safely. Support clients effectively to achieve optimum independence, interaction, social inclusion and own goals. Work as part of the overall Headway team in handling certain client and carer queries on finance. Liaise with Headway colleagues on individual client circumstances/changes. Input information about activity/cost onto the client database in line with agreed controls/procedure. Set up, prepare, clear away client and visitor refreshments. Undertake general cleaning duties necessary to maintain a high standard of customer service and environment.
