Job Description


We're looking for a skilled and enthusiastic team member whose role will support with the Wellbeing service commissioned programmes, including Early Years, Weight management, HENRY and Oral Health. Being able to engage with community groups and delivering sessions will be part of this key role. Also to engage with key partners in the community in order to be more sustainable and increase opportunities for healthy lifestyle changes in targeted areas.Main duties of the jobThe role will also support service leads to engage with local residents and partners to fully understand the communities needs for delivering education and awareness programmes and activities.About usAt CHCP, we're passionate about people and we know that you are too, that's why we're here isn't it?   • Knowledge of the health and social benefit of a healthy lifestyle
  • Knowledge of factors influencing behaviour and behaviour change approaches
  • An understanding of the barriers and challenges that people may face in connecting to and accessing local activities and support
  • An understanding of the importance of accurate record keeping and knowledge of information governance requirements
Desirable
  • An understanding of Partnership working
  • An understanding of the role of voluntary and community sector organisations and the contributions they make to the health and wellbeing of individuals
  • An understanding of CHCP, NHS, Local Authority, Health and other relevant agencies and structures
  • Understanding of health inequalities how healthy lifestyle choices impact on this.
  • Knowledge of health and safety requirements in relation to processes for risk assessment in relation to venues and home visits
QualificationsEssential
  • General education qualification for English & Maths GCSE standard or equivalent
  • Recognised Teaching / Training / Group Facilitation Certificate (e.g L3 PTLLS education and training) equivalent experience or willingness to undertake training and develop training kills
  • Behaviour change/ Health Coaching training or willing to undertake
  • L2 Food Hygiene
Desirable
  • Minimum of Level 2 Physical Activity for Children (Nationally Occupational Standards)
  • level of relevant experience
ExperienceEssential
  • Experience of working with people from different backgrounds and respect different lifestyles and views
  • Experience of delivering training or facilitating group sessions
  • Experience of working effectively with groups/individuals
  • Demonstrable experience working with community groups, voluntary organisations
  • Knowledge and experience of IT systems including the ability to use Microsoft office; Outlook, Excel, Word and Power Point

  • Experience of developing new practical initiatives relating to health
  • Experience of working as part of a team
  • Experience and ability to identify risk and assess/manage risk when working with individuals
