Job Description

As a York Community Response Team Service Co-ordinator -- you'll be coordinating care and actively shaping recovery journeys for patients, facilitating health care services to be provided to patients within their own homes.



Within this role, you are:



First point of contact for patients, families, and health & social care partners, ensuring smooth communication and support. Managing patient pathways, including scheduling assessments, daily visits, and discharge planning. Supporting the day-to-day running of the service, helping maintain efficiency and responsiveness. Working closely with clinicians and other professionals to deliver integrated care. Facilitating short-term rehabilitation and reablement to prevent unnecessary hospital admissions and support post-hospital recovery.

This is a linchpin role within a system designed to keep people safe, supported and independent in their own homes. You'll be helping reduce hospital strain while improving patient outcomes and quality of life.



The post holder will proactively support all CRT referrals, ensuring each stage of their pathway is fully coordinated, planned and booked. Will act as a main point of access to the service providing contact for patients, carers and wider healthcare team to provide (in conjunction with the clinical team), high quality patient care through proactive identification of needs which require a level of clinical knowledge, triage, allocation and discharge planning.



Collaborate and communicate with the wider health and social care team, acting as a resource to ensure the provision of appropriate services to maximise the health and wellbeing of the patient, improving the quality and efficiency of healthcare delivery. The post holder will be responsible for the coordination of the administrative function within the complex CRT pathways arranging assessments, reviews, discharge planning, transfers of care and multi-disciplinary team discussion in a timely manner, to facilitate unnecessary hospital admission and timely hospital discharge. Will be expected to manage and prioritise the allocation of support worker visits and work flexibly to meet the needs of the service, which can often be demanding and unpredictable.
