We are looking for a highly motivated Youth Link Worker to lead interventions and become an integral part of our team based at St George's Square in Huddersfield.
You will work as part of the Kirklees Social Prescribing Team. The Youth Social Prescriber will work directly with young people aged 11-25 to improve their health, wellbeing, confidence, and sense of connection within their communities. Using a person-centred and trauma-informed approach, the post holder will support young people to access non-clinical services, activities, and community resources that help them build resilience, develop positive relationships, and overcome social, emotional, or practical challenges.
They will be responsible for managing a busy caseload of young people, offering interventions, signposting and referring to local groups and establishing and maintaining partnerships with appropriate schools, organisations and individuals.
We require a wellbeing practitioner to work on one or more evening shifts (2:30 - 7:30pm) Mon-Fri
Duties
Provide one-to-one, person-centred sessions to explore the young person's needs, strengths, interests, and wellbeing goals.
Develop tailored social prescribing plans, connecting young people with relevant activities, groups, and services.
Support young people to overcome barriers to engagement, including confidence issues, practical challenges, or anxiety.
To work closely with community leaders to ensure Invictus' offer to them is robust and effective and that they are getting the service they want and need.
Under supervision, undertake accurate assessments of risk to self and others
Develop strong local knowledge of youth services, extracurricular activities, wellbeing programmes, and community assets.
To identify and report safeguarding incidents, in line with relevant Invictus Wellbeing's policies.
Provision of information, advice and guidance (IAG) to YPs and staff regarding related issues, typically these would include general welfare, academic, professional, health and personal.
Reporting on trends and needs in the above areas in order to improve and develop support for young people.
To update monitoring documents/reports consistently and analyse trends and use them where appropriate in support sessions.
To keep spreadsheets, administrative processes and client records up to date.
Ensure that confidentiality is always protected in accordance with confidentiality policy and procedures.
To manage busy caseloads ensuring effective communication with families, professionals, staff, CYPs and more.
To undertake ongoing professional development relevant to mental health and emotional wellbeing services.
To undertake other duties which are commensurate with the role.
Qualifications
Experience working with vulnerable populations including those with behavioural challenges or mental health issues is preferred
Valid driving licence
Strong understanding of medical terminology and patient care procedures
Excellent organisational skills with the ability to develop personalised care plans
Compassionate attitude with excellent communication skills and patience when working with young people facing diverse challenges

Pay: 23,800.00-27,550.00 per year
