Hospital Discharge Support Worker Organisation: Disability Direct
Location: Derby City
Salary: 12.85 per hour
Contract Type: Zero Hours
Driving Licence: Full UK Driving Licence Required
Role Overview
Disability Direct is seeking compassionate and dependable individuals to join our Hospital Discharge Team on a flexible, zero-hours basis. This vital role supports patients with disabilities and additional needs as they transition from hospital to home, ensuring they have the essentials and support required for a safe and comfortable recovery.
Key Responsibilities
o
Transport patients from hospital to home using a company or personal vehicle.o
Support patients with settling in, including switching on heating, preparing light meals, and collecting prescriptions.o
Deliver essential items such as groceries, medication, or equipment.o
Liaise with healthcare professionals, family members, and community services to ensure continuity of care.o
Observe and report any concerns about patient wellbeing.o

Maintain accurate records of visits and support provided.Requirements

Full UK driving licence and access to a reliable vehicle use of company vehicle if as well as owno
Excellent communication and interpersonal skills.o
Ability to work flexiblyo
Experience in care, support work, or community services is desirable but not essential.o
