Job Description

An exciting opportunity has arisen at Drive within in the My Life My Way Team.

Applications are warmly invited from candidates with an interest in supporting and enabling individuals with learning disabilities, physical disabilities, Autism and mental health to live their lives their way and encouraging full involvement in activities within the local communities.

You will create and lead opportunities for people to learn new skills, whilst accessing indoor and outdoor activities of their choosing. You will also be required to network with our community partners and local businesses.

We are looking for 2 x 20 hours posts:



to cover Bridgend & NPT



to cover Merthyr & RCT



Possession of a full UK driving licence and access to a vehicle is essential and it is an advantage to be familiar with Microsoft Office

You will be required to

:

1. Provide access to activities and learning opportunities during the day, evening and weekends. Work with the My Life, My Way team to:



Create and lead in- person and on-line activities.

Organise resources, venues and equipment for specific activities.

Promote Drive and external activities on social media.

Coach people we support to lead activities.

Liaise and work alongside organisations and partners.

Post holder will be allocated main area/s of work, but will be required on occasion to cover other areas.

2. Support to individuals



Provide high quality support promoting individual rights, choice and control, personal beliefs, and identity.

Provide practical and emotional support to enable people to enjoy a range of hobbies and activities, including volunteering or employment.

Promote communication skills and relationship building - supporting the individual to understand the world around them and promoting positive relationships.
Provide emotional supportto individuals in a sensitive manner with empathy and using active listening, offering advice and guidance enabling individuals to make informed decisions and exercise their rights.

Encourage and empower individuals to express their opinions and share their views in all aspects of decision-making.

Promote equality for all individuals through anti-discriminatory practice.

Ensure the health, safety and security of supported people, staff, visitors and the environment.

Act as an appropriate role model, when supporting individuals. Dress and act appropriately to participate in each daily activity.

Effectively communicate with families, staff and relevant services to maintain a high standard of support and provision of a quality service.

Provide individual's with effective information so they can make informed decisions and exercise their rights.

3. Personal Development



Complete the All-Wales Induction Framework and QCF Level II/Level III award in Health and Social Care.

Register and maintain registration with Social Care Wales.

Participate and contribute to 1:1 supervision and appraisal sessions as agreed with Line Manager.

Attend and actively participate in mandatory and relevant training events and keep accurate records of Continuous Professional Development (CPD).

Maintain accurate records of hours worked and the completion of timesheets.

Participate and contribute to relevant team and organisational meetings.

4. Administrative Responsibilities



Maintain accurate records of support e.g. activity completed, outcome for individual supported, financial transactions etc

4. Organisational Responsibilities



Ensure work practice is in line with Drive Policies and Procedures and industry standards.

Ensure decisions made under the Mental Capacity Act or Best Interest decisions are maintained and represent the least restrictive practice.

Ensure IT skills are sufficiently up-to-date to be able to utilise required IT systems.

This post is subject to satisfactory references , enhanced disclosure from the Disclosure and Barring Service (DBS), P.O.V.A check, health declaration and legality to work in the UK.
