Job Overview:
We are looking for a highly organised and motivated Administrative Support Officer to join our Primary Care Liaison Learning Disabilities Team . This vital role supports the coordination and delivery of services aimed at improving access to healthcare for individuals with learning disabilities across primary care settings.
You will work closely with clinical staff, GP practices, and external partners to ensure smooth administrative operations, accurate data management, and effective communication that underpins the team's work in reducing health inequalities.

Provide administrative support to the Primary Care Liaison LD Team, including diary management, meeting coordination, and minute-taking.
Maintain and update learning disability registers across GP practices.
Support the coordination of annual health checks and follow-up appointments.
Input, manage, and extract patient data using NHS clinical systems in line with data protection policies.
Assist in the preparation and distribution of accessible health information, including easy-read materials.
Act as a point of contact for internal and external enquiries, ensuring professional and timely communication.
Support the organisation of training sessions and health promotion events for primary care staff and carers.
Ensure all administrative processes align with NHS Wales policies and contribute to service improvement initiatives.
