We are excited to offer an Advanced Clinical Practitioner (ACP) post in Medicine for the Elderly (MFE) at University College Hospitals NHS Foundation Trust (UCLH).
This is our second Advanced Clinical Practitioner post within the team supported by the Acute Frailty consultant lead and Consultant clinical lead for MFE. You will also work closely with the multidisciplinary team and be further supported by the Medical Specialities Matron.
We are looking for a dynamic, inspirational, and forward-thinking health care professional. As a qualified ACP you will require physical assessment skills and non-medical prescribing to perform comprehensive geriatric assessments and liaise with community providers to develop holistic plans of care and treatment for our older adult population. We work to the ethos of recognising Frailty and enabling older people to maintain health, well-being, and maximum independence. You will be actively involved in developing and delivering our acute care pathways and providing Comprehensive Geriatric assessment reviews and care planning.
You should have extensive experience in the Care of the older people as well as having excellent leadership, management, and communication skills.
Enthusiasm, a desire to learn and flexibility are essential for this role, as are good interpersonal and organisational skills. You will take pride in your responsibility for the quality of the care in your clinical area, provide sound clinical and professional leadership to your multi-disciplinary team and create a culture where the team feel empowered. The service aims to deliver and sustain the highest standards of care, quality, safety and dignity for all of our patients.
