Job Description

Nova Healthcare Solutions Ltd is a CQC registered organisation that provides remote support to primary healthcare services through workflow management and medicines optimisation.

The ANP Clinical Lead role takes a senior position in the development of workflow processes, policy and protocols. The postholder acts as a key point of contact for customers and has ownership of completion, auditing and support of workflow and clinical services.

The postholder will themselves ensure that they and the staff undertake a variety of duties, including reviewing, highlighting, coding and actioning workflow documents.

The role is currently remote working. Full training is provided.

Clinical practice

To organise cover and participate in weekly remote triage sessions and "premium" and "premium+" workflow support. To assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis and complex needs. To clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly. To proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). To diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan. To prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national protocols. To work with patients to support compliance with and adherence to prescribed treatments and support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care. To provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions. To prioritise health problems and intervene appropriate to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. To support and manage health needs presenting for family planning, cervical cytology or sexual health consultation. To assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
Staff support

To work within set parameters to efficiently supervise staff in all aspects of workflow management and clinical services. To proactively create and deliver regular training materials and sessions to staff in line with company policy and protocols. To ensure that staff form a high performing team and meet all set key performance indicators and targets. To spot check an agreed quantity of staff work to ensure that the highest standards are maintained, flagging, retraining and resolving any issues encountered within a timely manner. To ensure that records are up to date and accurate. To ensure staff are compliant with mandatory training. To take a pro-active approach towards recruitment, induction and training of staff.
Workflow processing

To work within set parameters to ensure workflow documents have been uploaded to the correct patient records by the host practice and are legible. To appropriately code workflow documents using Emis and SystmOne codes. To follow company policies in managing workflow documents. To action any documents in adherence to company protocols. To ensure that workflow is processed in line with department key performance indicators and individual targets. To ensure workflow is processed to the highest standard in keeping with patient safety.
Administration

To effectively use the IT software provided, check emails daily and keep an accurate record of personal information, vaccination status, staff sickness and leave requests. To produce standard letters, emails and collect information using available IT packages in line with current procedures. To maintain and file accurately all data, documents and correspondence required for the operation of the service. To ensure that any claims for overtime are submitted, and other financial claim forms in line with current procedures. To ensure adherence to lone working procedures as required.
Customer care

To deliver care according to NSF and NICE guidelines. To utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment. To communicate with and support patients who are receiving bad news recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. To ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate. To carry out audits of work processed for organisations, addressing or highlighting issues encountered throughout the process. To provide clinical support with complaints and feedback, including providing clinical support in client meetings and taking the necessary action in consultation with the Senior Management Team. To contribute effectively to delivering a first-class customer focused service. To follow company complaint reporting procedures always taking ownership and highlighting any errors made. To ensure that every organisation has an audit completed every 3 months.
Service development

To work with the Executive Management Team to grow and develop services. To both contribute and encourage team contribution towards service improvement and achieving and maintaining CQC standards. To represent the company positively at all times in communication and meetings with professional and external agencies. To highlight instances of conflict between company procedures and CQC / client requirements. To address issues of quality, risk assess own performance and take accountability for own actions and those of their team either directly or under supervision. To work effectively with individuals in other agencies to meet patients' needs as required. To effectively manage own time, workload and resources.
Miscellaneous

To carry out efficiently all associated tasks to support the smooth running of the service that may be assigned to you by the Senior Management Team. Adhere to Disclosure and Barring Service Checks

Pay: 65,000.00-70,000.00 per year
