Hoveton and Wroxham Medical Centre is looking to recruit an advanced nurse practitioner/advanced clinical practitioner to join our multi-disciplinary team. This role is to provide acute "on the day" appointments as well as pre-booked appointments for patients with acute illness or ongoing problems.
The role is part-time for 20 hours each week working Wednesday all day, Thursday morning and Friday afternoon. This may involve an early start, 6.45am and late finish, 8pm and some Saturday working.
Your main place of work is Hoveton & Wroxham Medical Centre within the NN4 PCN but you may on occasion need to provide support to other PCN practices: Acle Medical Partnership, Brundall Medical Partnership, Blofield Surgery, Ludham & Stalham Green Surgeries, Stalham Staithe Surgery.
We are looking for an advanced clinical practitioner who can: Treat a variety of acute and chronic presentations.
Work independently to see patients in the surgery, in care homes and housebound patients.
Work well under pressure and as part of a team delivering high quality care.
Prescribe independently, safely and effectively.
The post-holder will:
Assess, diagnose and treat undifferentiated patients with a range of acute, non-acute and chronic medical conditions and minor injuries.
Initiate appropriate care plans including requesting necessary tests, onward referral to other health care professionals and agencies and follow up consultations.
Promote health and screening in relation to primary prevention strategies.
Facilitate innovative approaches to effective systems and processes for patient management.
Work with the clinical and management teams to deliver a timely, cost effective service.
