The post holder will be expected to exercise a high degree of personal professional autonomy and make critical judgements to satisfy the expectations and demands of the role and service. Assimilate highly complex strands of information autonomously, to reach a differential diagnosis, order appropriate investigations and begin first line treatments in line with local and national guidelines. Demonstrate comprehensive, efficient, timely and targeted clinical history taking. Able to liaise with community and Mental Health services to provide holistic assessment of these patients.
4. Demonstrate ability to perform a mental health assessment. Takes a focussed mental health and substance misuse history. Working autonomously, as a clinical expert: assess, plan, implement and evaluate specialist nursing provision.
Develop and implement specialist nursing protocols. Develop and maintain nurse led initiatives including clinics. Communicate sensitive condition related information to patients and relatives. Provide ongoing support and reassurance.
Assessment of incoming referrals. Undertake clinical, psychological and social assessment of patients. Interpret results, liaising with multi-professional colleagues where indicated. Maintain accurate documentation.
