An exciting opportunity has arisen for a B8a Advanced Practitioner for SEND to join CDC nursing in the London Borough of Barking & Dagenham NELFT. We are looking for someone who enjoys working as part of a skilled interdisciplinary team within our children's service, working collaboratively with our partner agency for the benefit of children with special educational needs and disabilities and their families'/carers'. Working with our local partners to ensure the local offer is reflective of service provision that is responsive to needs. This is an exciting time to join our children's service, and drive innovation for BCYP with SEND forward. This includes the development of neighbourhood MDT's and our own Proactive care pilot for children which has made significant impact on children with complex health needs accessing their education.
This is a full-time post with the opportunity for innovation in service provision working with the Consultant Nurse to ensure that Barking & Dagenham lead the way in meeting the needs of our diverse population.
