We are a compassionate healthcare provider dedicated to delivering exceptional patient care. We are seeking a skilled and empathetic Aftercare Nurse to provide phone-based follow-up care for patients. This role involves ensuring patients' recovery post-treatment, answering their medical questions, and offering emotional support in a professional and caring manner.

As an Aftercare Nurse, you will be responsible for providing high-quality care and guidance to patients over the phone following their medical procedures or treatments. You will assess their recovery progress, address any concerns or complications, and ensure they have the necessary information to continue their healing process effectively.

Key Responsibilities:

Conduct phone-based follow-up calls to assess patient recovery and well-being after treatment or procedures. Provide medical advice and answer patient questions regarding their aftercare, including medication management, lifestyle recommendations, and potential side effects. Identify any signs of complications or adverse reactions and escalate them appropriately. Offer emotional support and reassurance to patients, maintaining a compassionate and professional tone. Educate patients on best practices for a smooth and speedy recovery. Document patient interactions and any recommendations provided in the electronic health record (EHR) system. Collaborate with the broader medical team to ensure continuity of care.

Qualifications:

Active Registered Nurse (RN) license. Previous experience in nursing or healthcare, with a focus on post-operative or aftercare preferred. Strong communication skills and the ability to explain complex medical information clearly and compassionately. Ability to manage phone-based consultations with professionalism and empathy. Strong organizational skills and attention to detail. Basic proficiency with EHR systems and Microsoft Office tools.
