Part time & Full time across different locations:

Rockingham Forrest and Kettering Southwest Rural Primary Care Network- NN8, NN9, NN11, NN14, NN15, NN16, NN17, NN18, NN29, LE16, PE8 X 37hrs

Triangle Primary Care Network- NN14, NN18 X 15hrs

Grand Union Primary Care Network- NN1-NN5 X 22.5hrs

Duties



As an Ageing Well Coordinator, you'll be a key member of a multidisciplinary team, working closely with GP surgeries and community partners within PCN. Your role will involve:

Taking referrals from GP practices and arranging home visits Representing Northamptonshire Carers within the Ageing Well team Carrying out holistic assessments and patient observations Supporting people aged 65+ with complex long-term condition. Helping individuals avoid unnecessary hospital admissions or institutional care Promoting independence and wellbeing--helping people choose well, stay well, and live well in their own homes

Skills



Experienced in support work, wellbeing coordination or within health and social care Skilled in working collaboratively across multi-disciplinary teams Confident using electronic recording systems Knowledgeable about services available to support older people A great communicator--both written and verbal Empathetic, a good listener, and able to build trust with individuals and families Organised, self-motivated, and capable of managing a varied caseload independently
You may already be working as a primary care practitioner, within pcn primary care network or support worker

Pay: 24,295.43 per year

Expected hours: 15 - 37 per week
