Job Overview



We have an exciting opportunity for a dedicated, experienced ANP to join our friendly and well-established team of 4 GP Partners, 2 Salaried GPs, 2 Practice Nurses, 1 HCA, 1 Phlebotomist and reception/administration team serving 8700+ patients.

The ideal candidate will be an Independent Prescribing Nurse with experience in primary care.

The practice runs a total triage system and the post holder will play a crucial part in this process.

Clinical Practice



Undertake Clinical Nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to patients accessing primary health care services.

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting

with an undifferentiated diagnosis

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Undertake on the day triage, face to face and telephone consultations, and home visits as assessed by the GP's.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
