Blossom Health Gosport are looking to recruit a Prescribing Nurse Practitioner to join our innovative and cohesive clinical team.
Located on the ground floor of Gosport War Memorial Hospital we are a well established, expanding practice with a list size of just under 6000 patients. Recently rated Good by CQC across all areas, our clinical team consists of 3 salaried GP's, 1 prescribing ANP, practice nurse, respiratory nurse, diabetic nurse and a very experienced advanced HCA. As part of our expansions plans we are seeking to employ another Prescribing ANP for approximately 27hrs per week to support the clinical team.
We have an excellent working relationship with our neighbouring practices via our PCN with access to Pharmacists and Pharmacy technicians, First Contact Practitioners, Health and Well Being Coaches and Social Prescribers.
The Surgery has an incredibly friendly atmosphere and everyone has an opportunity to offer suggestions and voice opinions on the development of services.
Conduct comprehensive assessments of patients to determine their healthcare needs.
Develop and implement individualised care plans based on patient evaluations.
Prescribe medications and monitor patient responses to treatments.
Collaborate with multidisciplinary teams to ensure holistic patient care.
Educate patients and their families about health management and medication usage.
Maintain accurate and up-to-date patient records in compliance with legal and ethical standards.
Stay informed about advancements in medical practices, particularly in pharmacology and treatment protocols.
Skills
Independently assess, diagnose, and manage patients with acute and chronic conditions, following established protocols and guidelines.
Prescribe medications within your scope of practice and in accordance with relevant legislation.
Order and interpret diagnostic tests, such as blood tests, X-rays, and ECGs.
Develop and implement individualised patient care plans.
Provide health promotion and disease prevention advice to patients.
Maintain accurate and comprehensive patient records in accordance with organisational policies.
Work collaboratively with GPs, nurses, and other healthcare professionals within the multidisciplinary team.
Participate in clinical governance activities, including audits and service improvement initiatives.
Keep up-to-date with current evidence-based practice and guidelines.
