Job Purpose


The Assistant Care Co-ordinator will work under the guidance and supervision of a Senior Social Worker or Assistant Team Manager to support the delivery of high-quality, person-centred care management services. The role involves working directly with vulnerable adults to assess their needs, contribute to support planning, and help them achieve their maximum level of independence. The role also includes providing responsive assessments and interventions, particularly for individuals in crisis within the community.

Specific Accountabilities



Assist in the Care Management process in line with departmental procedures and policies, under the direction of a Senior Social Worker or Assistant Team Manager. Work in partnership with people who use the services and their carers, respecting their views and promoting participation in decision-making wherever possible. take responsibility for obtaining regular supervision, which includes evaluating practice, reflection and career development. Demonstrate an understanding of the legal and policy frameworks that inform adult social care practice. Undertake assessments and arrange support plans where needs are clearly defined Understand forms of harm and their impact on people and demonstrate a thorough understanding of Dudley's Safeguarding Procedures.

Person Specification



Experience of working in a health or social care setting, ideally with vulnerable adults. Understanding of the principles of person-centred planning and care Ability to work independently and as part of a team, using supervision appropriately.

Additional Information



Valid driving licence and access to a vehicle is essential. Enhanced DBS clearance will be required for this position.
Job Types: Full-time, Temporary, Fixed term contract, Temp to perm
Contract length: 5 months

Pay: 14.82-19.38 per hour

Expected hours: 37 per week
