We are seeking an enthusiastic and dynamic Band 4 Associate Practitioner to join our well-established and supportive multidisciplinary Falls Service.
As part of our multidisciplinary team, you will collaborate with physiotherapists, occupational therapists, nurses, and other professionals, ensuring that each patient receives a holistic approach to their care. You will also receive regular supervision and support from senior clinicians, who are committed to helping you develop your skills and expertise within the field.
It is essential that you can commute effectively across the Hull and East Riding geographical area, as the role involves traveling to various patient locations. Therefore, A full UK driving license and access to a vehicle are required due to the travel involved.
Throughout your time with the team, you will be supported in your professional growth, with access to a range of learning and training opportunities. We are looking for someone who is passionate about delivering high-quality patient care and working as part of a dynamic, supportive team.
Main duties of the job
You will play a key role in providing holistic assessment and interventions to patients who have experienced a fall, with the opportunity to visit patients in their own homes and care homes across the Hull and East Riding area. In this role, you will manage your own caseload of patients, delivering falls prevention and rehabilitation interventions under the supervision of senior clinicians.
You will be responsible for conducting comprehensive assessments, developing individualised care plans, implementing tailored interventions to reduce the risk of further falls and educating patients and their families about falls prevention strategies.
The service operates on a 7-day working pattern, and as part of the team, there is an expectation to contribute to this, including working on public holidays. If you are a dedicated and motivated individual looking to make a real difference in the lives of patients, we would love to hear from you.
