The Role of a Case Manager is to act as a single point of contact throughout the client journey. They are professionals who co-ordinate the care and rehabilitation a client may need following an ABI or other complex injury.
The Case Manager will work with a range of professionals, to bring together all of the therapies required and build a quality plan of care, working towards rehabilitation and allowing the client to live their best life.
You will review case progress and help the client to achieve wellness and independence as far as is possible.
A Case Manager will act as a liaison between clients and their treatment or care options. Co-ordination and management of the Multi-Disciplinary team (MDT) is one of the key functions. Therapists may include:
Neuro Physiotherapists, Neuro Psychologist, Neuro Psychiatrist, Occupational Therapy, Speech and Language therapy, Specialist Education provision and work environments to name just a few.
You will also work alongside Litigation teams, Court of Protection teams and appointed Deputies.
You will be supported by the senior leadership team to provide an excellent framework that enables our staff to deliver outstanding care to the clients.
We're looking for someone who can build effective professional relationships, works well within a team and within company and regulatory guidelines.
Your own car and a full clean driving licence is essential. This role requires an enhanced DBS check and is full time 37.5 hours per week. We do offer some hybrid working and flexibility to suit you and our families, with a salary of up to 39,000 - negotiable on experience.
