T has difficulties with higher level cognitive functions (processing, decision making, multi-tasking, problem solving, memory etc). She experiences high levels of fatigue and would benefit from support to manage the structure of her days and to engage in social & leisure activities outside of the home. She also needs assistance to manage the home and admin/planning. She may need assistance to attend appointments. She experiences high levels of anxiety, especially in community settings. T is mildly dysarthric and occasionally has difficulty making herself understood because of this.
In addition to her brain injury, T has mental health difficulties characterised by episodes during which she experiences paranoid ideation and/or auditory hallucinations.
