An exciting opportunity has arisen to join one of the GP practices in our Primary Care Network (PCN) to help administrate the delivery of care for our patients.
Based in Birchwood Medical Practice in Creekmoor and Oakdale, this friendly and supportive practice requires an administrator/Care Coordinator, to help the practice administratively in their day-to-day work ensuring patients have the best quality of care.
You will also help with our work across our PCN in the delivery of Covid vaccinations, population health management and any collaborative work we do as a group of practices to fully support the health management of our patients.
The ideal candidates need to be excellent communicators with interpersonal skills and be highly efficient administratively. You should Ideally have some understanding of primary care or have GP practice experience, but this is not essential as training will be given.
Job Role
Practice based work will include:
Providing general administrative support to the Practice Management team.
Workflow - Scanning and processing of daily post
Helping with Health campaigns, smoking cessation, flu and smear campaigns as well as others needed by the practice as and when required.
Monitoring the Net account
Liaising with carers and those who they care for
Liaising with the Patient Participation Group.
GDPR - ensuring we are compliant with relevant legislation on website.
Ensuring that practice staff can effectively care navigate patients to appropriate services.
Network based work will include:-
To help with any Network based health promotion work
To help with any Covid Vaccination work (including working at the Covid vaccine site, inputting of data, troubleshooting queries etc.)
To help our Lead Care Coordinator Team with any projects they may be undertaking
To help reach the various targets as dictated to the Network by NHS England, the clinical commissioning group etc.
And any other duties deemed reasonable by the Management team, Network Services Manager or Lead Care Coordinator.

Work to a high professional standard to avoid mistakes
Requirements are:
Have a valid and current driving licence
Experience of working in the NHS (advantageous not essential)
