Job Description

Care coordinators are an important member of the PCN multidisciplinary team (MDT) identifying patients in need of proactive support.

The role will require them to be able to work with and understand the roles of a variety of practitioners working within the PCN such as doctors, nurses, paramedics, physician associates, social prescribing link workers, physiotherapists and clinical pharmacists as well as the administrative teams.

The care co-ordinators will work closely with the primary care team to provide well resourced, integrated care closer to home to support patients in the management of their conditions, to remain in good health and to lead independent lives.

The post holder will also be involved in the smooth and efficient delivery of PCN vaccination programmes.

2. Key Duties & Responsibilities:



Care coordinators will:

Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids.

Bring together all of a person's identified care and support needs and explore their options to meet these into a single personalised care and support plan, in line with PCSP best practice.

Support the PCN care Home coordinators and care home leads to provide PCSP for residents in PCN aligned care homes.

Help people to manage their needs, answering their queries and supporting them to make appointments.

Support people to take up training and employment, and to access appropriate benefits where eligible through referral to social prescribing link workers.

Raise awareness of shared decision making and decision support tools and assist people to be more prepared to have a shared decision-making conversation.

Ensure that people have good quality information to help them make choices about their care.

Support people to understand their level of knowledge, skills and confidence (their "Activation" level) when engaging with their health and wellbeing, including through use of the Patient Activation Measure.

Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing.

Support the coordination and delivery of MDTs within PCNs.

Analyse various information as part of quality improvement projects.

Completion of any work streams agreed by the Chief Nursing Officer.

To act as a non-clinical cancer champion.

To act as a non-clinical carers champion.

3. Other Responsibilities



Health and Safety

To work within the Clinical Governance Framework of the practice, incorporating Risk Management and all other quality initiatives and all aspects of CQC implementation.

To maintain confidentiality of information relating to patients, clients, staff and other users of the services in accordance with the General Data Protection Regulations 2018 including outside of the work environment. Any breach of confidentiality may render an individual liable for dismissal and/or prosecution.

Safeguarding



Whilst in post, staff are expected to acquire and update their knowledge on safeguarding as per the intercollegiate document requirements and MMP policies.

Professional development



The post holder will participate in any training programme implemented by the practice as part of this employment

To participate in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development

General



To undertake any other duties commensurate with the role, within the bounds of his/her own competence as guided by the attached management framework.

To work across the various MMP sites as required.

In light of national policy and due to the needs of the business it may be necessary for the Partnership to alter the opening hours of the surgeries. This could incorporate different opening hours and weekend working which may effect when you are required to work. The post holder is expected to be flexible and accommodating, following consultation, in terms of any changes to operating times in the future.
