Job Description

We are seeking an exceptional Care Coordinator to join our dynamic team and make a meaningful impact on our local community. This role offers the opportunity to work at the forefront of primary care, supporting patients through their healthcare journey while contributing to innovative service delivery.

What You'll Do



Patient Care & Service Coordination



Book appointments and manage queries for PCN services including LARC, Physiotherapy, Mental Health, ASD and ADHD assessments, and Dietitian services Provide expert advice, guidance, and signposting to patients Coordinate care for a cohort of patients, navigating the support necessary for their personalised care requirements Ensure patients receive high-quality information to make informed choices about their care

Administrative Excellence



Provide comprehensive administrative support to clinical and non-clinical colleagues Monitor shared email inboxes and task groups to ensure timely responses to patient queries Manage referrals and ensure proper documentation and coding Support Quality and Outcome Frameworks (QoF) and national/local targets Provide accurate and timely performance reporting

Operational Support (20%)



Assist with back-office administration, including recruitment and onboarding processes Support operational delivery of Primary Care Network services and care pathways Contribute to PCN priority areas and directed initiatives Attend PCN meetings and collaborate with practice staff

What We're Looking For



Essential Requirements



Educated to GCSE or equivalent Exceptional communication skills with a clear, polite telephone manner Discretion and confidentiality - ability to work sensitively with patient information Public-facing experience with empathy and sensitivity in distressing situations Team collaboration skills with the ability to think quickly and adapt Strong IT and administrative capabilities Organisational excellence with attention to detail

Desirable Experience



Previous medical or healthcare receptionist experience Experience in primary care or NHS environments Knowledge of SystmOne

Serving a combined population of 70,000 patients, we provide healthcare to a diverse socio-economic and demographic community, creating an interesting and varied caseload.
