We have an exciting opportunity for a Care Coordinator to join us at our offices in Fife and west Lothian regions of Scotland. The Care Coordinator will effectively and efficiently manage the delivery of high quality care services, ensuring that Care Workers are matched to our clients based on the level and type of care they need, along with, when possible, being in close proximity. The Care Coordinator will support and empower our professional carers enabling them to to excell in their role.You will direct, mentor and supervise the team to ensure quality standards are maintained and that care is delivered in a timely way in line with the service users' individual needs and wishes.

Key Responsibilities:
To develop and maintain runs of work that are geographically planned, which use resources efficiently and continuously reviewed and updated as necessary.
To produce staff rotas on a weekly/fortnightly basis, using the organisation's computer based roster system. All records and rotas to be up to date at all times and be compliant to the organisational and legal requirements of a safe and well planned service.
To organise the care team whilst considering the changing needs of service users.
To promote dignity, respect and choice for service users; ensuring that the care team works within this ethos at all times.
Ensure that care and support is provided for service users in a timely way; upholding and improving standards of care delivery.
To ensure that all care staff engage in the support and care of the service user; promoting independence that enhances wellbeing and quality of life.
To have a good understanding of individual service users' needs and be able to respond effectively and responsively to changing needs.
To ensure all care is delivered to the highest possible standard and that service users' emotional and wellbeing of service users at all times.
To contribute to the care planning process for individual service users by sharing information with the Senior Care Staff about their needs, wishes and any changes that you have noted.
To adhere to all safeguarding processes and make sure that they are always followed.
To monitor the care practice of individual carer's, addressing any issues of concern proactively, including reporting and documenting as required.
Ensure compliance with all relevant health and safety regulations
Skills:
Excellent organisational and time management skills
Strong attention to detail
Effective communication and interpersonal skills
Proficient in using CarePlanner software and other computer systems
Knowledge of health and social care procedures, terminology, and correct practices
Ability to multitask and prioritise tasks effectively
Our ideal candidate will have:
Previous experience of one or more years working in a fast paced Home Care Service environment
Ability to communicate effectively
Exceptional organisational skills and attention to detail
Previous experience with rota planning and allocating care staff
Achieved Level 2 in Health and Social Care or equivalent
Ability to work under pressure and to meet deadlines
Ability to problem solve and work using own initiative; multitask; prioritise tasks
Knowledge and understanding about Care Inspectorate expectations and other health and social care regulations
Driving licence and own vehicle is essential
