The role will be working with our Reablement and Rapid Response Teams, accepting / processing referrals and developing call lists for the staff who already have an established rota's. You'll liaise with social workers & hospital discharge planners daily.

Shifts are scheduled between8:30am and 8:30pm, with some weekend working required on a rota basis.
An Enhanced DBS Check is required.
