Job Description

Care is Central is looking for a vibrant, passionate and organised Care Co Ordinator to join our Independent Living Scheme at All Saints View in Houghton Regis due to the growth of the service.

All Saint's View is an established independent living scheme in the centre of Houghton Regis. The scheme houses individuals aged 55 and over and is comprised of a mixed community of individuals living independently, and others requiring varying degrees of care and support

As a Care Co Ordinator you will play crucial role within this busy scheme ensuring the people we support receive timely and appropriate care in line with their needs, wishes and preferences. The Care Coordinator will lead on the organisation and deployment of staff to cover care visits, allocating carers to service users and effectively using resources across the scheme whilst balancing the preferred visit times of those we support.

Main Duties of a Care-Co Ordinator



To manage the service rota daily ensuring all client visits are scheduled in accordance with individual care plans and staffed appropriately.

To ensure that a minimum of four weeks' rota is prepared at any given time to ensure the service is fully prepared.

To respond quickly and positively to ensure there is sufficient staff cover and to reallocate calls when required in the times of staff sickness and absence.

To visit potential new clients in their own homes to complete initial care needs assessments and prepare detailed assessment reports for the registered manager.

To write initial care plans and risk assessments based on the assessments of need at the commencement of new care packages.

To liaise with the brokerage and adult social care teams to effectively negotiate packages of care for both new and existing clients, always advocating for the needs of the clients to ensure positive outcomes. To ensure detailed records are maintained and accessible regarding confirmation of care packages and any changes to these.

To undertake on-call duties for the service on a rota basis for which an additional payment will be made monthly.

Requirements



A minimum of one year working in an adult social care role, preferably with some entry level management experience (e.g. senior carer, shift lead, care coordinator).

A positive, calm demeanour with a 'can-do' attitude and a willingness to help others and find solutions.

Excellent partnership working skills and ability to work in collaboration with a range of internal and external partners, agencies and stakeholders.

Excellent communication and negotiation skills across a range of formats such as written and verbal communication.

Possession of a relevant professional or practice related qualification such as a Level 2 or 3 Diploma/NVQ Where candidates do not have a qualification, they must be willing to work towards these
