Job Summary


The Care Coordinator Service Delivery Lead is an important member of the PCN multidisciplinary team (MDT) and Care Coordinator Team.

The role will require them to be able to work with and understand the roles of a variety of practitioners working within the PCN and maintain a good understanding of QoF/DES/LES.

The Care Coordinator Service will assist and support care co-ordinator staff training, delegation of duties, monitoring of CCO Staff and act as a main point of contact within the team.

The post holder will be responsible for completion of any work streams agreed by the Clinical Delivery leads.

2. Key Duties & Responsibilities:



Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles

Bring together all of a person's identified care and support needs and explore their options to meet these into a single personalised care and support plan, in line with PCSP best practice

To work on improving and correcting data quality and read codes, and ensure all patients are contacted and offered appointments in line with their

Work closely with the Clinical Delivery Leads MMP sites / areas are focused on with regards the achievement of clinical targets

Assist in the creation and implementation of new starter training plans and support the on-boarding of new CCO staff

Manage / assist with the delegation of workload and priorities

Manage annual leave, sickness and rota management - including clinical rota's to support the clinical delivery leads.

Line management of Care Co-Ordinators

Arrange regular meetings with the Care Co-Ordinators

Provide support to the team by delivering regular 121's, annual appraisals and feedback

To regularly attended any meetings arranged by the Clinical Delivery Leads to ensure all communication updates are received and assist in the distribution minutes/action points after these meetings

To provide input to the Clinical Delivery Leads producing and evaluating monthly reports on the position of the organisation around clinical targets

Analyse various information as part of quality improvement projects and to help the team to provide a robust service to maximise the income potential
offered through Clinical target initiatives & PCN DES

Work closely with the clinical delivery leads to develop strong working relationships with a variety of stakeholders, both internal and external, e.g., Clinicians, Site Lead Officers, CSU Clinical system facilitators, Care home managers and external organisations.

Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles.

Support the coordination and delivery of MDTs within PCNs.

Co-ordinate and lead on quality improvement projects analyse and feedback information clinical delivery leads.

3. Other Responsibilities



Health and Safety



To comply with the Health and Safety at Work etc. Act 1974.

To take responsibility for their own health and safety and that of other persons who may be affected by their own acts or omissions.

To ensure all staff are fully trained and conversant with fire regulations and health and safety and understand what to do in an emergency.

Ensure a trained first aider is available in the building.

Ensure fire alarms are tested regularly and within the set guidelines.

Whilst in post, staff are expected to acquire and update their knowledge on safeguarding as per the intercollegiate document requirements and MMP policies.

General



To undertake any other duties commensurate with the role, within the bounds of his/her own competence as guided by the attached management framework.

To work across the various MMP sites as required.

In light of national policy and due to the needs of the business it may be necessary for the Partnership to alter the opening hours of the surgeries. This could incorporate different opening hours and weekend working which may effect when you are required to work. The post holder is expected to be flexible and accommodating, following consultation, in terms of any changes to operating times in the future.

Pay: From 13.00 per hour

Expected hours: 37.5 per week
