Job Summary:



We are seeking a compassionate, organized, and proactive

Care Co-ordinator

to join our team. The Care Co-ordinator plays a vital role in ensuring individuals receive the right care and support at the right time. This position involves working closely with clients, their families, health and social care professionals, and other stakeholders to develop, implement, and monitor personalized care plans that promote independence, safety, and well-being.

Key Responsibilities:



Serve as the main point of contact for service users and their families. Conduct comprehensive assessments of client needs, including physical, emotional, social, and environmental factors. Develop and regularly review individualized care/support plans. Coordinate care services across multidisciplinary teams, including GPs, nurses, therapists, and social workers. Monitor and evaluate service delivery, ensuring high-quality standards are met. Maintain accurate and up-to-date records in line with regulatory and organizational policies. Respond promptly to changes in client needs, updating care plans and communicating with relevant stakeholders. Advocate for clients to ensure their voices are heard in decision-making. Support discharge planning and transition of care between services or providers. Participate in team meetings, supervision, and ongoing professional development. Ensure compliance with safeguarding policies and procedures.

Requirements:Essential:



Proven experience in a health, social care, or support coordination role. Knowledge of care planning, risk assessment, and care delivery systems. Strong interpersonal and communication skills. Excellent organizational and time management abilities. Ability to work independently and as part of a multidisciplinary team. Understanding of safeguarding and confidentiality. Proficient in using care management systems and Microsoft Office.

Desirable:



Qualification in Health and Social Care, Nursing, Social Work, or related field (e.g., NVQ Level 3/4, Diploma, or Degree). Experience working with specific populations (e.g., elderly, mental health, Disabilities). Driving license and access to a vehicle.
