Job Summary


The Care Coordinator plays a pivotal role in ensuring that patients receive comprehensive and coordinated care. This position involves working closely with healthcare providers, patients, and their families to facilitate effective communication and streamline the care process. The ideal candidate will possess strong organisational skills and office experience, enabling them to manage various administrative tasks while prioritising patient needs.

Responsibilities



Responsible for the management of Rota co-ordination Coordinate patient care by liaising between healthcare providers, patients, and families to ensure seamless communication. Schedule appointments, follow-ups, and necessary tests for patients. Maintain accurate and up-to-date patient records in compliance with privacy regulations. Assist in the development of care plans tailored to individual patient needs. Provide support to patients and families by answering queries and offering guidance on available resources. Monitor patient progress and report any concerns to the appropriate healthcare professionals. Collaborate with multidisciplinary teams to enhance the quality of care provided. Oncall duties included on a rota basis

Skills



Proven office experience, demonstrating strong administrative capabilities. Excellent communication skills, both verbal and written, with the ability to convey information clearly and compassionately. Strong organisational skills with attention to detail, ensuring all tasks are completed efficiently. Ability to work collaboratively within a team environment while also being self-motivated. Proficiency in using office software and electronic health record systems is advantageous. Empathy and understanding towards patients' needs, fostering a supportive environment.
Job Type: Full-time

Pay: 25,000.00-28,000.00 per year
