As we expand, a fantastic opportunity has arisen for a full-time Care Coordinator to join our team which is based in Wrexham. We are regulated by the Care Inspectorate Wales (CIW) and strive for excellence in compliance and service delivery.
Purpose
As the Care Coordinator, you will be responsible for scheduling all domiciliary care workers rota for their designated area. Alongside this, you will be providing a high standard of person-centred care to our clients in their own homes.
Duties and responsibilities
Help to manage the day-to-day operations of our domiciliary sector
Monitor the quality of the service as directed by quality monitoring systems
To undertake the day-to-day monitoring of work standards of a team of care workers, including specialist teams where appropriate, referring performance and disciplinary issues to the Registered Manager
To assist with maintenance of records, in written or computerised format, as specified within procedures or as requested by care coordinators. Specifically, to ensure that documentation in the homes of service users complies with Care Cymru policies and procedures and in adherence to care standards
Attend service user reviews as required
Complete care rotas for staff and service users ensuring that all daily care packages are covered on a weekly basis
Deal promptly and appropriately with any complaints received
Participate in any managers/staff meetings putting items forward for the agenda
To provide cover in other areas as and when required
To participate in the out of hours on call 'back up' service as required on a rota basis
Person specification

Experience and Skills:
Good communication and diplomacy skills, knowledge of risk assessing and care planning.
Completion (or willingness to successfully complete) QCF Level 3 or above in Health & Social Care or equivalent qualification.
Ability to work flexibly depending on the needs of the service.
Experience working as a Care Coordinator is essential.
Be highly organised
Work well under pressure
Own vehicle/ drivers' licence is essential
Knowledge and understanding:
Excellent understanding of the needs of people who require care and support at home and the provision of homecare services in line with best practice
Excellent understanding of the principles of high-quality person-centred care and support and non-discriminatory care practice
Understanding of systems to maintain confidentiality in relation to service users, staff and the business
Knowledge of how to recognise abuse and safeguarding procedures
