Job summary



We are recruiting a Care Co-ordinator to work at Eastfield Medical Centre

The role will suit an enthusiastic individual who is able to work collaboratively across the team and be keen to deliver within the evolving role of the network.

You will work with both patients and healthcare professionals to ensure seamless, organised, and effective care.

The ideal candidate will:

have experience of working with people with complex social and health needs,

be committed to supporting individual decision making and a person-centred approach,

have experience of working with voluntary and community organisations.

Be able to adjust at short notice the needs of the role.

Main duties and responsibilities of the job



Working closely with the Practice team

To support the social prescribing team obtaining information from patients and signposting services.

To support existing projects to provide cover as and when required.

To provide administrative support for various projects as and when directed.

To keep accurate records using agreed systems and contribute to project evaluation.

Achieve agreed performance targets and participate in regular supervision and personal reviews.

Engage in training, identify personal and professional development needs and continuously seek to refresh knowledge in relation to the job.

Cooperate in the formulation and development of systems for monitoring and evaluating the work of the organisation.

Provide information for and contribute to the administration of communications media as appropriate.

Where required and appropriate refer people back to other health professionals within the primary care network.

Support staff within the Practice information about local voluntary and community resources and how patients can access them.

Treat with confidentiality any information about any organisation, its staff, trustees or clients that is sensitive, personal or private.

Cover the work of colleagues during holidays or absences.

Safeguarding:



Be responsible for and committed to safeguarding and promoting the welfare of adults-at-risk (vulnerable adults), children and young people and for ensuring that they are protected from harm.

Be aware of your duties and responsibilities regarding current legislation and adhere to our policies and procedures on Child Protection and Safeguarding Adults.

Recognise the types and signs of abuse and neglect.

Ensure the Safeguarding Lead is alerted to, and kept fully informed of, any concerns you may have in relation to safeguarding adults, children and young people in a timely manner.

Stay up to date with safeguarding training requirements as per business guidelines and professional registrations etc.

Learning and development:



You will participate in any training programme implemented by the Practice as part of this employment, such training to include:

Participation in individual performance reviews, including maintaining a record of own personal and/or professional development.

Working in conjunction with senior management, assess own learning needs and undertake learning as appropriate.

Develop and utilise a written Personal Development Plan.

Make effective use of learning opportunities within and outside the workplace, evaluating their effectiveness and feeding back relevant information.

Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments.

Undertake mandatory and statutory training as required.

Team working:



Assist and support your line manager to provide a quality service.

Provide cover for members of your team during periods of sickness and annual leave.

Support and facilitate change as requested.

Undertake additional tasks as required within your general role.

Understand own role and scope in the organisation and identify how this may develop over time.

Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.

Accept delegation from senior staff, prioritise own workload and ensure effective time management strategies are embedded in own practice.

Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team's performance.

Participate and support local projects as agreed with the management team. Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence of those taking on delegated duties.

Person Specification



Qualifications

Essential

Achieved grade C or above, in English and Maths GCSE or equivalent.

Desirable

NVQ Level II (Health and Social Care) or equivalent experience.

Formal training working with vulnerable individuals.

Experience

Essential

Previous experience working with the general public.

Experience of working autonomously and part of a team.

Experience of working with vulnerable individuals

Desirable

Experience of working with individuals with long term conditions.

Evidence of working within a multidisciplinary team.

Experience of working without direct supervision.

Knowledge and Skills

Essential

Ability to recognise and respond appropriately to risk and safeguarding concerns.

Knowledge around importance of confidentiality and data protection.

Good communication and interpersonal skills, including an ability to build rapport and establish good one to one relationship.

Ability to deal with challenging behaviour and difficult conversations.

Be able to offer support in a person centred and non-judgmental way.

Ability to effectively manage a variable workload.

Ability to maintain accurate and concise records.

Ability to provide information effectively.

Good IT skills and proficient in the use of various Microsoft packages.

Additional Attributes

Essential

Willingness to work and travel across all Practice's within the PCN.

Have a full, clean driving license and have access to a car during all contractual hours.

Ability to work flexibly in an innovative and developing role.

Job Type: Part-time

Pay: 13.50 per hour

Expected hours: 25 per week
