We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will play a vital role in ensuring that service users receive the highest quality of care by facilitating communication between healthcare providers, patients, and their families. This position requires strong organisational skills and the ability to manage multiple tasks efficiently while providing exceptional support to our clients.
Responsibilities
Serve as the primary point of contact for clients and their families, addressing inquiries and concerns with empathy and professionalism.
Coordinate client care by scheduling appointments, managing referrals, and ensuring timely follow-up on treatments.
Maintain accurate and up-to-date patient records, ensuring compliance with relevant regulations and confidentiality standards.
Collaborate with healthcare professionals to develop and implement individualised care plans tailored to each patient's needs.
Monitor client progress and communicate updates to relevant stakeholders, including family members and medical staff.
Assist in the development of educational materials for clients and families regarding care options and resources available.
Participate in team meetings to discuss patient cases, share insights, and contribute to continuous improvement of care coordination processes.
Skills
Strong organisational skills with the ability to manage multiple priorities effectively.
Excellent communication skills, both verbal and written, with a focus on building rapport with patients and families.
Proficient in office administration tasks, including data entry, scheduling, and record management.
Ability to work collaboratively within a multidisciplinary team while demonstrating a commitment to patient-centred care.
Empathetic approach with a genuine desire to support individuals in navigating their healthcare journey.
Familiarity with healthcare systems and terminology is advantageous but not essential
Service specific information/ benefits
This role is suited to someone who is organised and keen to learn/ progress within the care sector.
You will be based at our Stafford Office - Monday to Friday 09:00-17:00. Our service covers all of Staffordshire and we are recruiting due to expansion.
You will work alongside skilled managers and co-ordinators to support a team of carers who currently deliver 1700 hours of care per week.

- This role will involve being involved in our out of hours rota, you will be paid additional to this and can earn approximately 200-400 extra per month.
