About the Role:

As our

Home Care Coordinator

, you'll be the friendly, dependable connection between our clients, caregivers, and healthcare providers. You'll manage client care plans, match caregivers with clients, monitor service quality, and keep everything running smoothly behind the scenes--so our clients get the care they need, when they need it.

What You'll Do:

Develop and manage individualized care plans Coordinate caregiver schedules and client visits Communicate regularly with families, caregivers, and medical providers Ensure compliance with care standards and regulations Provide compassionate support and solutions to clients and their loved ones Oncall one weekend and 5 days in a month

What You Bring:

Experience in home care, healthcare coordination, or case management Strong organizational and communication skills Empathy, patience, and a solutions-first mindset Comfort with scheduling software, EMRs, and multitasking Must hold a full driving licence

Pay: From 29,000.00 per year

Schedule:

10 hour shift 12 hour shift 8 hour shift Flexitime Monday to Friday Overtime Weekend availability
Experience:

care co ordination: 2 years (preferred)
