Job Overview


We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that customers receive the highest quality of care by coordinating services and facilitating communication between customers, families, and healthcare providers. This position requires strong organisational skills and the ability to manage multiple tasks efficiently while maintaining a customer - centred approach.

Responsibilities



Coordinate care services for customers, ensuring that their needs are met in a timely manner. Serve as the primary point of contact for customers and their families, providing support and guidance throughout the care process. Maintain accurate records of customer information, support plans, and service delivery. Collaborate with healthcare professionals to develop and implement effective care strategies. Monitor customer progress and adjust support plans as necessary to ensure optimal outcomes. Provide education and resources to customers and families regarding available services and support options. Assist in resolving any issues or concerns that may arise during the care process.

Qualifications



Previous office experience is desirable, demonstrating strong administrative skills. Excellent communication skills, both verbal and written, with the ability to engage effectively with customers, families, professionals and care support workers. Strong organisational abilities with attention to detail to manage multiple cases simultaneously. Empathy and understanding of the challenges faced by customers in need of care services. Proficiency in using computer systems for record keeping and communication purposes. Ability to work independently as well as part of a collaborative team environment.
