About the Role





We are looking for a motivated and compassionate Care Coordinator (with Health Care Assistant duties) to join our friendly and supportive St Helens South Primary Care Network (PCN) team.



This is a varied and rewarding role combining clinical responsibilities with administrative and care coordination support.



As part of our multidisciplinary team, you will work alongside GPs, nurses, pharmacists, and other healthcare professionals to deliver high-quality patient care and ensure the smooth running of our network practices.

What You'll Do





In this hybrid role, your primary focus will be providing Health Care Assistant support, including:


Taking blood samples (phlebotomy) Measuring blood pressure, pulse, height, and weight Performing ECGs Supporting clinicians with patient preparation and basic clinical tasks Maintaining clinical areas and ensuring infection control standards are met




You will also provide secondary support to the Care Coordination team, helping with:


Patient signposting and general enquiries Appointment and prescription requests Data entry, coding, and administrative support Rota assistance and reception cover Supporting MDT meetings and communication across the PCN




This is a fantastic opportunity for someone who enjoys working in a patient-facing role but also values teamwork, organisation, and variety in their day-to-day work.

About You





We're looking for someone who is:


Experienced in working as a Health Care Assistant or similar role Competent in performing routine clinical tasks such as BP checks, bloods, and ECGs Friendly, approachable, and patient-focused Organised and confident using IT systems Able to work effectively as part of a multidisciplinary team Flexible and willing to support colleagues across the PCN when needed Experience in primary care or a PCN environment is desirable, but full training and support will be provided for the right candidate.
