We are proud to offer the opportunity for a Care Navigator Supervisor to join our successful social prescribing team embedded within Wirral 0-18 CYP MHS. This is an opportunity to work as part of a multi-disciplinary healthcare team supervising non-clinical staff who provide 1:1 personalised guidance to children, individuals and families utilising a holistic and strength-based approach to co-produce strategies that address what matters to the child.



The post holder will provide clinical oversight and support to a team of care navigators monitoring standards and developing staff through a robust and regular clinical supervision.



They will be able to manage the coordination of complex care needs engaging appropriately with Education, Health & Social Care Teams and the voluntary sector to reduce avoidable crisis while young people wait to be allocated for therapeutic interventions.



They will be able to offer clinical reviews of young people's mental health to support the wellbeing of young people whilst they are waiting, co-ordinating and escalating with team leads when necessary.



They will work to support the integration of Trust and wider Wirral strategic priorities e.g SEND & Transitions.



They will act as operational link worker to statutory and non-statutory Partners; providing consultation to colleagues about CYP with SEND or other needs to support understanding of young person and provisions available in community. As part of this area of the role, network with Mental Health Support Team (MHST) SEND focused practitoners to maintain a holistic view of service responses/provision.



Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire.
