Care Navigator
Job Description

Nant Dowlais Health Centre opened on 1 July 2020, serving a 10,000 patient list in West Cwmbran. We are seeking an additional Care Navigator to expand our team.

5 weeks of annual leave
Shifts will be as follows:
Monday 1230-1830
Wednesday 0800 -1300
Thursday 0800 -1800
Friday 0800 -1300
The successful candidate will also be required to cover annual leave and sickness at short notice when needed.
