We have opportunities for two newly created posts as Band 4 Care Navigators to join the Shropshire, Telford and Wrekin CAMHS services.
The purpose of these newly created roles is to offer early intervention and health promotion to children and young people. This may involve working with families within their homes and office/clinic base.
You will need to have NVQ Level 3 Health & Social Care or similar or demonstrate equivalent level of experience plus knowledge of Mental Health acquired through formal training or experience.
You will be supporting families, carers and young people throughout the mental health referral process. You will support and signpost families, carers and young people in order to meet their needs and to ensure they access the right intervention through signposting to partnership services.
You will work within a multidisciplinary team, and liaise with wider multidisciplinary colleagues including educational settings, local authority and charitable and voluntary organizations, to promote health and reduce inequalities for children, young people and families.

Provide guidance to Family and carers in order for them to access timely support and understanding of the referral and assessment pathway process.
Support the team to ensure that families are kept informed of options available to them
Support children, young people and family to understand the MH services across the CAMHS system
Be involved with signposting and discharging children and young people
Meet regularly with supervising clinicians and manager
Work independently with children, young people and families
Follow the internal processes if a child or young person's care needs indicate review for assessment
Advocate views and choices expressed by child, young person and family.
Contribute to the assessment of and adhere to risk management under direction of senior clinicians.
Adapt in response to changing needs and circumstances of service users and carers, to show empathy and sensitivity with regards to social situations and difficult family circumstances
Access regular child protection supervision and work to trust safeguarding policy
Participate and contribute to team meetings and relevant professional forums.
Attend regular meetings with the multidisciplinary team
Undertake mandatory training and other learning opportunities identified in appraisal
Ensure information is available for children, young people and families in a range of formats.
Ensure outcome measures are routinely used and collated appropriately.
Participate in projects (Facilitate groups)
