Role Overview
We are seeking an experienced
Care Needs Assessor
with strong knowledge of
Birdie
to support our clients in the community. This role is key in assessing care requirements, developing bespoke care plans, and ensuring excellent care outcomes through accurate and up-to-date digital records.
Key Responsibilities
Conduct comprehensive care needs assessments for elderly clients, in person or remotely.
Create and update digital care plans and risk assessments using
Birdie
.
Liaise with families, GPs, and social workers to build holistic, person-centered care packages.
Review care plans regularly to ensure they reflect changes in client health, mobility, or environment.
Respond to urgent assessment requests and changes in care needs promptly.
Support care teams by ensuring assessments are clear, accessible, and actionable in the Birdie app.
Assist in CQC inspections, audits, and safeguarding reviews with well-documented evidence.
Champion digital best practices in assessment and care planning.
Requirements
Minimum 2 years' experience in care needs assessment, ideally in elderly or domiciliary care.
Essential:
Daily working experience with
Birdie
software for assessment and care planning.
Sound knowledge of Elderly and complex care issues: frailty, dementia, mobility challenges, and medication support and Learning Disabilities.
Excellent interpersonal and communication skills.
Organized, tech-savvy, and committed to continuous improvement.
Full UK driving license and access to a vehicle.
Desirable
Experience working with CQC-registered services.
Additional training in dementia care or end-of-life care.
Previous experience mentoring or training others in using Birdie or other digital care tools.
NVQ Level 3 or 4 in Health & Social Care (or equivalent)
