What you'll be doing:
As a Care Needs Assessor, you will play a key role in delivering high-quality, person-centred care to elderly and vulnerable people at our Residential Care Home, Heath Lodge. You will lead on the assessment and review of care and support needs, working closely with residents, families, and our care team to ensure dignity, independence, and wellbeing remain at the heart of everything we do. Your role will be vital in shaping tailored care plans, ensuring accurate documentation, and supporting compliance with CQC standards and our internal quality framework.
What we need you to bring to our community:
You should have a minimum level 3 qualification in Health & Social Care and strong experience in assessment or care planning within a residential or health care setting. Most importantly, you will be a compassionate, confident, and proactive individual who communicates clearly, works collaboratively, and is committed to improving the lives of those in your care.
