About the Service
The Carers Support Service is a brand new service designed to provide short-term, high-quality support to the person being cared for, enabling unpaid carers to take a much-needed break from their caring responsibilities. This vital service recognises the immense contribution unpaid carers make and aims to reduce the risk of carer breakdown, improve wellbeing, and help carers maintain their own health, relationships, and independence. By offering reliable, flexible carers breaks, the service plays a key role in sustaining informal care arrangements and preventing crisis.
Responsibilities
To provide high-quality, short-term support to individuals in their own homes, stepping into the unpaid carer's role during pre-arranged breaks, often visiting multiple homes in one shift.
To deliver care and support as outlined in the cared-for person's coproduced care and support plan, with the primary aim of enabling the unpaid carer to take a meaningful and worry-free break.
To assist with agreed domestic tasks, light cleaning, or household duties as part of the cared-for person's daily routine--fostering independence while maintaining continuity in the carer's absence.
To support individuals with personal care, mobility, nutrition, and other daily living tasks for the duration of the carer's break only, in line with the outcomes set out for the carer.
To safely prompt or administer prescribed medication according to the individual's plan, adhering to Nurtrio's medication policy.
To assist the cared-for person with shopping, meal preparation, or cooking where required--maintaining routines as expected by the carer and supporting wellbeing.
To promote inclusion and wellbeing by engaging the cared-for person in appropriate social or therapeutic activities, where agreed, helping to maintain routines during the carer's break.
To recognise and respond to safeguarding concerns promptly, maintaining vigilance while providing temporary care on behalf of the carer.
To accurately record details of support provided using the Nourish electronic care planning system, noting any changes in wellbeing that may affect the carer's ability to continue their role.
To work both independently and collaboratively, depending on the needs of the cared-for person and service delivery arrangements.
To provide a flexible service, generally operating between 8am-8pm, 7 days a week, with some carers breaks delivered outside these hours by prior arrangement.
To provide reassurance and clear feedback to carers following each break, including updates on how their loved one was supported and any relevant wellbeing observations.
To understand and support the use of assistive technology as part of the cared-for person's support plan, where appropriate.
To uphold and apply the principles of the Mental Capacity Act--supporting the cared-for person to make choices and participate in decisions that affect them, even during short-term support.
To support carer wellbeing indirectly, by being a trusted, reliable presence that allows unpaid carers to rest, recharge, or reconnect with their own life outside of the caring role.
