Medical Notes Summariser/Clinical Coder Report to: Clinical Administrator Accountable to: Partners/ Practice Manager Hours: Part time Job Summary Summarise and Read Code relevant information from patients clinical records into the Practice computer system, working strictly in accordance with the Practice notes summarising protocol. Provide general assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone. Key Responsibilities Summariser Input information from correspondence on to Patient Medical Record Culling and sorting patient records and letters in preparation for summarising Reviewing medical records and producing an accurate summary of the patients medical history. Read Coding the information into the Practice clinical system - Auditing data collection standards in the Practice - Monitoring progress of notes summarising against Practice targets - Monitoring patient call and recall systems by Liaising with other staff to ensure review clinics are booked - Coding & merging from GP2GP Collation of Information Support the Practice team in achievement of Enhanced Service and Quality Points targets by ensuring the following : Collation of information using EMIS medical information system.
Assist with the management of disease registers. Supporting GPs & mangers in prescribing Audits Taking action on CEG Prevalence report Assisting GP in CKD audit and Electronic referrals Administration Scanning All incoming clinical letters and faxes in patient notes Checking & Coding e-Documents & All discharge summaries and Clinical Documents & Faxes Maintaining RIP(Rest in Peace) Register, Coding & Taking appropriate actions Checking Practice generic emails and taking action Checking workflow manager daily and taking action Performance/Professional Development To participate in training sessions to ensure administrative and computer skills are kept up to date with current technology and practice. Attending all clinical meetings To participate in regular appraisal meetings Any Other Duties Any other duties as may be determined by the Practice Manager and Partners.
