Job Description

An exciting opportunity has arisen for a registered health care professional to work as Clinical Operations Manager, managing and leading the Out of Hours Community Nursing Team within the Intermediate Care and Urgent Community Response Directorate within NCH&C.



The Out of Hours team operates between 20:00-08:00 to deliver high standards of health care to patients within their homes across Norfolk and Waveney by assessing, planning and implementing urgent unplanned care.



You would be responsible for providing clear operational leadership and line management to the Out of Hours Team, which consists of a team of Nurses and Support Workers. The successful candidate will be responsible for the professional leadership of the Team, demonstrating a supportive and flexible leadership approach and a proactive, responsible attitude to promote confidence in the team and the service.



The post requires a confident, competent clinician with experiencing in managing a team and the challenges that this can bring. Experience of community working preferred but not essential.



The working day will be Monday-Friday with that will require flexibility in order to meet the needs of the service between the service operating hours.



To provide clinical leadership and line management to the Out of Hours Service.

To support staff to achieve and ensure competencies are maintained within the teams.

To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics, by assessing, planning, implementing and evaluating packages of care.

To ensure that Trust annual priorities and quality goals are embedded within the workforce by ensuring that all annual appraisals are complete within the team.

1. To provide professional and clinical leadership to the Out of Hours Service demonstrating a supportive and flexible leadership approach and a proactive, responsible attitude so as to promote confidence in the team and Service.



2. To operationally manage the Out of Hours Service across NCH&C.



3. To work within the integrated team to prevent unnecessary admission to hospital and facilitate early discharge form hospital.



4. To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.



5. To support community clinicians and students through supervision, appraisal and mentorship in line with trust policy.



6. To oversee that clinical supervision is occurring within the team and accurately recorded.



7. To provide professional clinical leadership to the Integrated Community Team within own sphere of practice



8. Operate on a four-week rolling rota no weekend working requirement.



As part of operational leadership for the locality you will be required to:



To deputise on a rotational basis for other COMS and leads including working in the operational hub for the hub leads.



Oversee the allocation of work for clinicians.



Ensure all staff are competent with mobile working.



Engage and lead on referral to discharge standard processes.



Work to a demand and capacity model which will reflect workload needs at any given time.
