Job summary:

This is an exciting opportunity to join our business development team as an Assessment Coordinator in delivering a bespoke high-quality assessment and onboarding process to our clients. We are seeking an open-minded, proactive and solution focused individual(s) to join a growing team within our Organisation.

The Business Development team will be busy with a high number of referrals from different local authorities, integrated care boards, and private clients that we will need to assess. We also work closely with discharge teams in setting up quick discharge to assess placements which will need a quick turnaround.

The successful candidate will be working in a fast-paced environment and will need to be an individual who can thrive in group settings and can work independently as well. The successful candidate will play an integral role in collating key documents to inform the support guidelines to be put in place for keeping the people we support safely.

This is a full-time position with a 6-month probation period.

Main duties of the job:

Review all supported living referrals workings alongside the business development team. Liaise with the placement teams and brokerage teams is arranging assessments and discharge. Carry out assessments for people referred to our services. Prepare assessments reports and gather enough literature to allow the MDT to agree on discharge plans and support levels.
Person Specification Qualifications (Essential)

Evidence of strong communication, organisation and administrative skills Experience working with adults with Learning Disabilities, Mental Health needs and or complex care with challenging behaviors. Able to work independently and collaboratively in a fast-paced environment. Knowledge of relevant legislations including Care Act 2014, CQC regulations, and Dols. Driving License
Desirable

Professional Qualification in Health and Social care, Business, related field and or willing to work towards. Experience of pathways for Mental Health and Learning disabilities Previous experience working in a supported living environment
